Family Swim Minor Participant Waiver
All minor children (anyone under 18 years old) wishing to swim during Family Swim must be accompanied by a YWCA member. For those wishing to swim with a member who is NOT the minor's parent or legal gardian, this form must be completed, signed, and turned in at the Membership Services desk prior to entering the pool area.

Temporary Legal Guardianship Form
Complete and return this form if you are not a current member and a family member or friend who is not a guardian of your child would like to enroll your child up for any activities or lessons at our facilities.
